[Notice] Notification of Change in Personal Information Controller and Transfer of Personal Information & Medical Records
As the comprehensive transfer of business was completed as of June 3, 2026,
we hereby notify you of the change in personal information controller and the transfer of personal information in accordance with Article 27 (Transfer of Personal Information Following Transfer of Business, etc.) of the Personal Information Protection Act as follows.

3. Transferred Personal Information Items and Purpose of Use
Transferred Items: Name, date of birth, gender, contact information, medical records (all medical records including treatment history, prescriptions, etc.), reservation and consultation history
Purpose of Use: To provide medical treatment, reservation management, and healthcare services under the same purposes as previously collected
4. Succession of Prepayments and Deposits
All prepayments and deposits made at the existing Obliv Clinic will be transferred to
Obliv Songdo Horizon Clinic.
Even after the change, they can be used under the same conditions as before,
and there will be no expiration or disadvantage regarding the remaining balance.
5. Continuity of Treatment and Services
Even after the change of representative and name, the existing medical programs, reservation systems, and aftercare services will be maintained without any disruption.
Existing customers can visit and receive treatment in the same manner as before without any separate procedures.
6. Right to Refuse Transfer and Data Subject Rights
If you do not wish for your personal information and medical records to be transferred, you may request to refuse the transfer.
Refusal Request Deadline: Until September 30, 2026
How to Apply: Complete the Personal Information Transfer Refusal Request Form or submit via email at obliv.contact@gmail.com
Please Note:
If you do not express your refusal within the deadline, the transfer will proceed.
Please note that refusing the transfer may result in limitations regarding future reservations and continuity of medical treatment.
Even if you refuse, medical records that are required to be preserved under relevant laws such as the Medical Service Act may be stored for the legally mandated retention period.
We will do our utmost to continue providing safe and satisfactory medical services to our patients.
